Stanford has a common end-of-quarter evaluation form for almost all schools.

End-of-quarter evaluations are mandatory, and starting this quarter they are
administered online. Midterm evaluations are voluntary and can take several
forms. For more information, check
http://ctl.stanford.edu/Faculty/evals_fac.html In my opinion, the process
works quite well at Stanford. We'll see how the online evals affect it!
Peer evaluations are not common at Stanford.
